Aging Well Advocates, LLC is a private, comprehensive Aging Life Care Management practice, serving older adults and their families in the Metro West area of Massachusetts, located in Franklin. Founded in February 2018 by Daniela Masters (MSW, LICSW, CMC), Aging Well Advocates was born out of the desire to provide a more personal touch to helping seniors and their families navigate the complex challenges of aging.

Daniela's philosophy for Aging Well Advocates is simple: Provide the unbiased support, guidance and advocacy to ease client's minds through what can be an unsettling time, with the overarching goal being to AGE WELL.

An Aging Life Care Manager is a PARTNER

• Helps direct your family to available services and housing options for short and long term planning
• Eases the overwhelming emotions associated with caring for the elderly, feeling “burned out” or confused about choosing the right care solutions
• Steps in with nursing care options when your loved one is unable to live safely in his/her current environment due to single or multiple physical, medical, or psychological issues
• Offers relief when your family has limited time and/or expertise in dealing with a loved ones’ chronic care needs, such as dementia caregiving
• Provides an objective professional assessment when your family is at odds regarding care decisions and needs
• Advocates for your family when experiencing suboptimal care with current care providers

An Aging Life Care Manager Provides SUPPORT

• Is an objective 3rd-party removing emotional issues which may complicate care when family members are caring for the elderly
• Acts as a local manager to maintain accountability for care when family members do not live nearby
• Professionally coordinates and communicates between all care service providers
• Coordinates appointments and medical follow up
• Advocates with medical professionals during hospitalization, rehabilitation or nursing care
